Community Health Worker

Our Mission:

The mission of The Midwife Center is to promote wellness by providing exceptional, client-centered primary gynecological, pregnancy and birthing care in southwestern Pennsylvania's only indepenent birth center.

Job Description:

The Community Health Worker (CHW) will increase access to care at The Midwife Center and in the community. This individual will meet with clients in person and contact them between or after scheduled appointments to supplement clinical care. The aim of these encounters is to connect TMC clients to community services and resources while providing additional health education and peer social support. The CHW will also colalborate with TMC clients and staff to ensure an integrated and personal approach to care. The position is grant-funded.

Position Type: Full-time (1.0 FTE)

Duties & Responsibilities:

  • Evaluate client needs and provide referrals to appropriate community resources and TMC services.
  • Establish relationships with clients to provide general support and referrals to meet immediate needs and health goals
  • Familiar with community and in-house resources relevant to client needs, with ongoing management and updates of the resource referral list.
  • Provide consistent communication with the program coordinator and with other staff as needed regarding client status evaluations.
  • Regularly seek supervision to review interventions and address client needs.
  • Cross-train and provide support in the front office as needed.
  • Collaborate with community organizations to build partnerships that benefit TMC and its clients.
  • Support the Community Engagement Coordinator with outreach events as appropriate.
  • Other duties as assigned to fulfill the mission of TMC.

Qualifications:

  • Post high school education preferred
  • Community health worker certification preferred
  • Public health training preferred
  • A minimum of two years relevant experience (such as home-visiting, perinatal doula care, clinical resource coordination, etc) preferred
  • Experience in care planning and customer service

Salary and Benefits

  • Salary: $47,000.00 per year. This position is grant-funded.
